29 CFR 1910.30(a)(1): Portable and powered dockboards were not strong enough to carry the load imposed on them. On or about August 14, 2012 in the small ticket production area of the facility; a). Employees were using cracked and damage portable wood dock boards/plates for powered industrial trucks, electrical conveyor rollers and employees to travel over to load and unload the products from the back of the tractor trailer.

29 CFR 1910.29(a)(3)(vii): Work levels 10 feet or higher above the ground or floor did not have a guardrail of 2" x 4" nominal or the equivalent, installed no less than 36" or more than 42" high, with a mid-rail of 1"x 4" nominal lumber or equivalent: On or about August 14, 2012 in the order picking and pulling production areas of the facility; a). The employees using the mobile ladder stands with work a platform level at 12 feet 5 inches from the ground level were not provided midrails to protect the employees from a fall hazard.
