29 CFR 1910.22(a)(1):  All place(s) of employment, passageways, storerooms, and service rooms was not kept clean and orderly and in a sanitary condition:    On August 14, 2012, and at times prior thereto,at the Walmat Store #0945 Deli area located in the south west center part of store where frying was being performed there was accumulated grease on the floor that made it slippery.  Employees were exposed to slips/trips/fall hazards.

29 CFR 1910.22(b)(1):  Aisle(s) and passageway(s) was not kept clear and in good repairs, with no obstruction across or in aisle(s) that could create a hazard:    At the Walmart Store #0945 located in the south center part of store and for a period of time up to and including August 14, 2012, the aisles and passageways in:   a.  Maintenance Storage Room  b.  Maintenance Department  c.  Storage Area   d.  General Merchandising/Receiving area  e.  Fixture Room  f.  Candy Room   were blocked with stored merchandise and equipment.  Employees were exposed to slips/trips/fall hazards.

Employees were exposed to entrapment hazards.

On August 15, 2012, and at times prior thereto, in the Automotive shop employees were using air hoses where handles had been attached with screw clamps and/or air supply was blocked by a pair of vice grips.   Exposing employees to struck-by hazard.
