On or about Friday, August 17, 2012 at 122-02 Roosevelt Avenue, 2nd floor, Flushing, NY 11366 Employees working on the 2nd floor of the construction project were exposed to a fall hazard of approximately 10 feet to the lower level as a result of unsecured floor hole covers.
