On or about Friday, August 17, 2012 at 122-02 Roosevelt Avenue, 2nd Floor, Flushing, NY 11366 Employees were observed working alongside an unsecured floor hole cover on the 2nd floor of the building exposing the employees to a fall hazard of approximately ten (10) feet to the lower level.
