Jobsite located at 16561 S. Kaw St, Olathe, KS, employee(s) accessed a roof using an extension ladder that had a broken, right rung lock and a bent, bottom rung, which had not been marked and removed from service, exposing employees to a fall hazard of approximately 13 feet to the ground below.
