OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees such as the employer fails to make necessary repairs, resulting in the operation of the cranes with defective or missing parts affecting the safe operation of the cranes and likely to result in serious injury or death in the event of a crane malfunction or failure:    (a) Fabrication Bay, #1 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial #5409K, hoist serial #143625): On or before August 20, 2012, employees operated the crane reported deficiencies to move materials prior to evaluating the severity and making a determination if the crane was safe to use until defeciencies could be repaired:    (b) Fabrication Bay, #2 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial # 5409K, hoist serial # 143628): On or before August 20, 2012, employees operated the crane with reported defeciencies could be repaired,    (c) Fabrication Bay, #3 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial #5409K, hoist serial #143020): On or before August 20, 2012, employees operated the crane with reported defeciencies to move materials prior to evaluating the severity and making a determination if the cranewas safe to use unitl defeciencies could be repaired.    (d) Fabrication bay, #4 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial # 5409K, hoist serial # 143623): On or before August 20, 2012, employees operated the crane with reported defeciencies to move materials prior to evaluating the severity and making a determination if the crane was safe to use unitl defeciencies could be repaired.     (1) The pendant festoon is cut, taped, and needs replaced.    (2) The trolley contactor is making excessive noise.    (3) Two collector shoe inserts are worn.     (4) There are no trolley bumpers.    (b) Fabrication Bay, #2 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial #5409K, hoist serial #143628): On or before August 20, 2012, employees operated the crane with the following deficiencies and without first conducting a frequent inspection:     (1) The pendant cord pulled out of the junction box.    (2) Two collector shoe inserts are worn.    (3) The pendants strain relief is broken.    (4) The gearbox needs to be flushed out and replaced with new oil. The oil is dirty.    (5) The load brake needs to be evaluated, the customer said the hoist is dropping its load.    (6) The wire rope needs replaced. There are several broken strands.    (7) There are no trolley bumpers.    (c) Fabrication Bay, #3 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial #5409K, hoist serial #143020): On or before August 20, 2012, employees operated the crane with the following deficiencies:     (1) Pendant cord is cut, spliced, taped, and needs replaced.    (2) There are no trolley bumpers.    (3) The drive chain is stretched and it doesn't appear to have any adjustment left. The pinion also slides on the shaft.     (4) The trolley gearbox leaks and needs resealed.    (d) Fabrication Bay, #4 Top Running Bridge, Single Girder, Under Hung Hoist (crane serial #5409K, hoist serial #143623): On or before August 20, 2012, employees operated the crane with the following deficiency:     (1) The trolley gearbox is leaking into the electrical cabinet and needs resealed.     The employer, Shane Felter Industries, Inc., was previously cited for a violation of this Occupational Safety and Health Administration OSH ACT of 1970 Section (5)(a)(1), which was contained in inspection #310291000, Citation #01, Item #001, issued on August 9, 2007, and became a final order on April 9, 2009, with respect to a workplace located at: 1569 Pittsburgh Road, Uniontown, Pa 15401.
