29 CFR 1910.106(b)(6):  In locations where flammable vapors maybe present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition:    a.  On or about August 18, 2012, employees, working on the roof of tank #1, were patching the tank roof iwth steel plates and screws.  During the operation, the flammable vapors inside the tank ignited, exposing the employees to fire and explosion hazards.
