29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) In the Warehouse Building - A fabricated metal platform used for elevating personnel, no identifiable manufacturer name, model, or serial number, had been used by maintenance personnel to service racks and lighting fixtures from the forks of a battery-operated Yale stacker, model RDE03G654I088. There was no capacity rating on the platform.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available after employee(s) have received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. (a) Establishment -- Employee(s), namely first aiders at this facility, were not offered the Hepatitis B vaccination series after each employee received the training required and witihin 10 working days of initial assignment to all employees who have potential occupational exposure in the event of a worker injury.
