29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a)  124 South Essex Avenue, Orange, NJ.    Employee was exposed to falls of 20 feet to the ground below from the tubular welded frame scaffold's third tier as the corner area of the scaffold platform adjacent to the access ladder was not provided with guardrails or other forms of fall protection and midrails were missing along the entire platform.                             Violation observed on or about 08/13/2012.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a)  124 South Essex Avenue, Orange, NJ and at the Corporate Offices at 1840 Englishtown Road, Old Bridge, NJ.    Employees were exposed to falls from the tubular welded frame scaffold as workers had not received instruction from the employer on, but not limited to, scaffold fall protection requirements.                               Violation observed on or about 08/13/2012.
