29 CFR 1926.502(h)(1)(iii): The safety monitor was not on the same walking/working surface and within visual sighting distance of the employee being monitored. On or about August 21, 2012, on the ground, the employer did not ensure that the safety monitor was on the same walking/working surface and within visual sighting distance at all times. The safety monitor was observed on the ground and traveling back and forth from exterior to interior of jobsite handling an orange electrical extension cord. This created a condition which exposed employees to the hazards of falls from an elevation of approximately 12-feet from roofline to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets On or about August 21, 2012, at the jobsite, the employer did not ensure that employees working and walking on low-slope, flat roof, with unprotected sides and edges of the roof, were provided a personal fall arrest system or other fall protection system, exposing employees to the hazards of falls from an elevation of approximately 12-feet. The (Bray Sheetmetal Company) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 315277020, citation number 1, item number 1 and was affirmed as a final order on (July 5, 2011), with respect t a workplace located at 10618 Breckenridge Drive, Little Rock, Arkansas.
