On or about August 21, 2012, on the ground, the employer did not ensure that the safety monitor was on the same walking/working surface and within visual sighting distance at all times. The safety monitor was observed on the ground and traveling back and forth from exterior to interior of jobsite handling an orange electrical extension cord. This created a condition which exposed employees to the hazards of falls from an elevation of approximately 12-feet from roofline to the ground below.
