29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating machinery:    BVM Corporation, at 430 S Navajo Street, Denver, CO:  On and preceding 8/21/12, BVM Corporation did not ensure that machine guarding was provided to protect operator(s) and other employees from hazard(s) created by rotating machinery.  One employee operated a Fosdick Machine Tool Co drill press while fabricating carbon steel parts.  The rotating chuck and head of the drill press were not guarded.  The employee worked in close proximity to the rotating parts while adding cutting fluid and monitoring the work.  The employer did not ensure that a machine guard protected the employee from the rotating chuck and head of the drill press.  This condition exposed the employee to a laceration hazard. 29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):    BVM Corporation, at 430 S Navajo Street, Denver, CO:  On and preceding 8/21/12, BVM Corporation did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2).  Employees performed tasks at a metal tool manufacturing facility.  Employees during the course of their duties were exposed to flying particulates, noise, intense light, and respiratory hazards.  The employer had not performed a written certification of the workplace hazard assessment.
