29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Job site, 1606 Constitution Road, Arnold, PA: On or about August 16, 2012, during roofing activities on the main house roof, the employer did not ensure that side rails of the ladders used to gain access to/egress from the roof were extended at least 3 feet above the roofline. The ladder used for this purpose was extended approximately 30 inches above the roofline, exposing workers to falls of up to approximately 20 to 25 feet.     b) Job site, 1606 Constitution Road, Arnold, PA: On or about August 16, 2012, during roofing activities on the porch roof, the employer did not ensure that side rails of the ladders used to gain access to/egress from the roof were extended at least 3 feet above the roofline. The ladder used for this purpose was extended approximately 18 inches above the roofline, exposing workers to falls of up to approximately 15 feet.

Deficiencies noted during the inspection included, but were not limited to, not ensuring that ladders extended at least 3 feet above the roof line and not using fall protection and not using head protection.
