29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 247 Main Street - On or about 21 August, 2012 - Subcontracted employees were using an extension ladder to access and egress from the roof that was not extended a minimum of 3 feet above the landing. Employees were exposed to a fall hazard of 12 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) 247 Main Street, Cairo NY - On or about 21 August, 2012 - Subcontracted employees were installing roofing shingles without properly wearing personal fall arrest systems. Employees were exposed to a fall hazard of 12 feet. F.H. ALEXANDER INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILIAR STANDARD, 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313760597 CITATION NUMBER 01, ITEM NUMBER 001, ISSUED ON 10/08/2010 AND FINAL ORDER DATE -11/01/2010
