29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, (a) John Wayne Bay - On or about August 21, 2012 employees were exposed to fall hazards of seven to nine feet when working near unguarded edges on the dredges. Employees were not protected from falling by using a guardrail system or personal fall arrest system.

All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure. (a) North Bay - On or about August 21, 2012 an employee was blowing off clothing using compressed air that exceeded 30 psi.
