29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous.      On or about August 20, 2012, west side of the construction site, employees of Mr. Efrain Dimas Ortiz utilized an extension cord with a missing ground pin to energize a radio and a Makita battery charger exposing employees electrical shock hazards.
