29 CFR 1910.269(d)(2)(iii): Procedures were not developed, documented, and/or used for the control of potentially hazardous energy covered by paragraph (d) of this section: Instances including, but not limited to the following: a) Jobsite, Hatfield Power Plant, 2907 East Roy Furman Highway, Masontown, PA: On or about November 15, 2012, during throat seal repair/replacement activities on the # 1-105 AR pump, the employer did not ensure that the procedures it developed and documented for energy control were followed in that employees performed this work with tags on valve handles, such as flush/discharge valve, and no extra steps, such as removing the valve handle or restraining it from being moved, were taken, allowing them to be potentially operated while employees did their work, enabling employees to be sprayed with water during this work. b) Jobsite, Hatfield Power Plant, 2907 East Roy Furman Highway, Masontown, PA: On or about August 20, 2012, during clearance restoration activities on the # 2 Silo Vent fan, the employer did not ensure that the procedures it developed and documented for energy control were followed in that while an employee was trying to install fuses into a disconnect panel, another employee was in the process of trying to start the Unit 2 Fly Ash System, potentially exposing the employee to an electric hazard.

29 CFR 1910.269(d)(2)(viii)(B): Retraining was not conducted whenever a periodic inspection under paragraph (d)(2)(v) of this section reveals, or whenever the employer has reason to believe, that there are deviations from or inadequacies in an employee's knowledge or use of the energy control procedures: Instances including, but not limited to the following: a) Job site, Hatfield Power Plant, 2907 East Roy Furman Highway, Masontown, PA: On or about November 15, 2012, during throat seal repair/replacement activities on the # 1-105 AR Pump, the employer did not ensure that energy control retraining was conducted. Employees performed this work with tags on valve handles, such as the flush/discharge valve, and no extra steps, such as removing the valve handle or restraining it from being moved, were taken, allowing the valve to be operated, enabling employees to be sprayed with water. b) Job site, Hatfield Power Plant, 2907 East Roy Furman Highway, Masontown, PA: On or about November 15, 2012, during maintenance activities on Precipitator unit # 3, the employer did not ensure that energy control retraining was conducted where deviations from appropriate energy control procedures occurred. Employees entered the unit for work and the control room operator placed an electronic tag on the computer system in the control room and no other actions were taken, enabling the louvers to be moved and change air flow direction while personnel worked inside.

29 CFR 1910.269(d)(6)(iii): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from energy sources: a) Jobsite, Hatfield Power Plant, 2907 East Roy Furman Highway, Masontown, PA: On or about October 8, 2012, during the Unit 2A clinker grinder area, impeller revoval activities, the employer did not ensure that all sources of potentially hazardous energy were isolated. Employees performed this work on the upper half of the "A" side of the Clinker Grinder Breaker tagged out, but other energy isolating devices, such as the ash gates, were not tagged, potentially exposing employees to hazards such as scalds/burns.

29 CFR 1910.269(d)(6)(iv): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a) Job site, Hatfield Power Plant, 2907 East Roy Furman Highway, Masontown, PA: On or about October 8, 2012, during grinder unjamming/debris clearing activities, the employer did not ensure that all sources of energy had lockout or tagout devices attached to them. Employees performed this work on the upper half of the "A" side of the grinder with only the grinder electrical breaker locked out, but other energy isolating devices, such as the ash gates, were not locked out or tagged out, exposing employees to hazards such as scalds/burns.
