29 CFR 1926.404(b)(1)(iii): 29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: At the site, located at 5940 Gulf Freeway, employees were using a Makita Circular Saw ( MN 5007MG SN 0267932) which had a damaged cord, exposing employees to electrical hazards.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: At the site, located at 5940 Gulf Freeway, employees involved in concrete work, were using electrical equipment with damaged cord sets. This occured in two instances; a) a yellow 50' 12/3 SJTW 20 amp flexible cord, and b) a Makita 7 1/4" circular saw (MN 5007MG SN 0267932). These conditions exposed employees to potential electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the site, 5940 Gulf Freeway at the free standing Chik-fil-a, employees were using a flexible cord to supply power to a Makita Circular Saw ( MN 5007MG, SN 0267932) that did not have strain relief, exposing employees to potential electrical hazards.
