29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      On or about August 20, 2012 on the west side of the construction site, employees engage in roofing activities on a residential building with a 6:12 pitch did not use fall protection.  The employees were exposed to falls of 9 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.      On or about August 20, 2012 on the west side of the construction site, employees engage in roofing activities on a residential building with a 6:12 pitch utilized an extension ladder that did not extend 3 feet above the roof exposing employees to a 9 foot fall.
