29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (l): (a) Parallon Supply Chain Solutions-Denver @ 4520 Florence Street. Denver, CO. On or before August 21, 2012, the employer did not ensure that each employee who operated the Crown PE-3500 Rider Pallet Truck in the warehouse was provided with operator training as required under the standard. This condition exposed employees to the hazard of unsafe operation to injury or death from lack of appropriate power industrial truck operators training. Abatement Note: Operator's training shall consist of formal instruction (e.g., discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by trainee), and evaluation of the operator's performance in the workplace. All operator training and evaluations shall be certified and conducted by a person who has knowledge, training and experience to train and evaluate the operator's competence.
