29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) Jobsite, 1501 Brigthon Road, Pittsburgh: On or about August 16, 2012, during HVAC removal and replacement activities, the employer did not ensure that employees used fall protection systems/devices where exposed to falls of up to approximately 20-25 feet.

29 CFR 1910.333(b)(2): While any employee was exposed to contact with parts of fixed electric equipment or circuits which had been deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph: a) Jobsite, 1501 Brigton Road, Pittsburgh, PA: On or about about August 16, 2012, during HVAC upgrade replacement activities, the employer did not ensure that employees were protected against electrical shock. While disassembling the old unit, the circuit breakers in the basement were shut off, but they were not locked out/secured against having them turned back on.
