29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), (a) The employee performing flagging duties had no traffic safety vest as described above and instead wore a white T-shirt and blue jeans. (b) The employee performing flagging duties had no "Slow/Stop" sign as described above and instead used a plain orange flag.

29 CFR 1926.404(b)(1)(iii): Where ground-fault circuit interrupters were not utilized, employer did not establish and implement an assured equipment grounding conductor program on the construction site covering all cord sets, receptacles which are not part of the building or structure, and equipment connected by cord and plug which were available for use or used by employees: (a) There was no assured equipment grounding conductor program that addressed the two extension cords used to bring power to the shop vacuum.
