29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  Roasting Room; On or about August 21, 2011 and at time prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work on the roasting oven which has multiple energy sources such as two electrical disconnects and a gas valve, without those employees having a written machine specific lock out procedure to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the roaster.

Bolts, nuts, and setscrews will, however, be permitted where they are covered with safety sleeves or where they are used parallel with the shafting and are countersunk or else do not extend beyond the flange of the coupling.    a)  Mill Area; On or about August 21, 2012 and at times prior thereto the employer exposed employees to fracture and amputation hazards in that employees were allowed to walk and work around exposed rotating motor and pump coupling at the peanut butter vat area.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    a)  Roaster Control Panel; On or about August 21, 2012 and at times prior thereto the employer exposed employees to electrical hazards in that employees were allowed to work around an operators control panel with an unused opening in the panel that was not effectively closed.    b)  480 Volt Breaker Panel in Roaster Room; On or about August 21, 2012 and at times prior thereto the employer exposed employees to electrical hazards in that employees were allowed to work around a breaker panel box with an open breaker slot exposing  live 480/277 volt AC buss bars inside the electrical panel.
