29 CFR 1910.26(c)(3)(i): A simple rule for setting up a ladder at the proper angle is to place the base a distance from the vertical wall equal to one-fourth the working length of the ladder. On or about August 14, 2012, the metal extension ladder used by an employee to gain access to the upper level of an apartment complex was not placed at an angle equal to one-fourth the working length. Employees using the ladder were exposed to fall hazards.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: Stairs or a ramp were not provided where the exit route was not substantially level. On or about August 14, 2012, the employer did not ensure the maintenance shop was equipped with an adequate means of exit. The sole means of entrance/exit was not substantially level and not equipped with a ramp or stairs.
