29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     Employees were climbing more than two feet from the scaffold working surface to the roof without a ladder or other means of safe access.

29 CFR 1926.451(e)(6)(i):     Integral prefabricated scaffold access frames shall be specifically designed and constructed for use as ladder rungs.    Employees were climbing mason type pass through frames to access the working level of the scaffold.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    Employees re-roofing a church were not protected from falling from the gable end of the roof while tearing off and installing roof jacks.
