29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Cam Machine C-10 area - On or about August 23, 2012 a 6-outlet power strip (relocatable power tap (RPT)) was used to power a hand-held Dremel rotary tool and a drill press. The power tap was energized and in use at the time of the inspection.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: ASSEMBLY AREA: On or about August 23, 2012, one wall-mount listed electrical box with knockouts (labeled as Cir 7 LUP 5) was used to provide pendant power. Flexible cords were observed to be attached to the electrical box was being used to energize a power hand tool. The 4 outlet box with knockouts was energized and sitting on the desk.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Rope Assembly Area: In July 2012 a new process was introduced to the facility which utilized braided ropes treated with urethane pre-polymer containing 2,4-toluene diisocyanate and cyclohexanone. No training was provided to the employees regarding the physical and health hazards (2,4-toluene diisocyanate may be a carcinogen) of these chemicals prior to working with the ropes.
