29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On August 23, 2012, outside the east end of the building, employees were exposed to struck-by hazards from falling stones, while working underneath and around other masonry workers on a 2-level scaffold, without wearing hardhats for protection.

29 CFR 1926.451(h)(1): In addition to wearing hardhats, each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects: On August 23, 2012, employee(s) were exposed to a struck-by hazard of being struck in the head by falling stones, as they were working underneath and around a 2-level welded scaffold, with a 5-foot section missing a toeboard for protection.
