Trucks with a fuel system leak or any other safety defect shall not be operated.    a) An employee was observed operating a powered industrial truck with an inoperable back-up alarm. The powered industrial truck is an Eagle TE-330M, Serial number: S-L1-23752.    b) An employee was observed operating a powered industrial truck with an inoperable back-up alarm and horn. The powered industrial truck is a Hyster S155XL2, Serial number: C024V02666D.    c) An employee was observed operating a powered industrial truck with an inoperable back-up alarm. The powered industrial truck is a Yale GC155CANGBE098, Serial number: B879V01523B.    d) An employee was observed operating a powered industrial truck with degraded tires.
