29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose: a. On or about August 16, 2012, the powercord on the MAXX Air electric floor fan was damaged and repaired with duct tape, exposing employees to an electrical hazard. b. On or about August 16, 2012, the powercord on the MAXX Air electric floor fan was damaged and repaired with black tape, exposing employees to an electrical hazard. c. On or about August 16, 2012, the powercord on the MAXX Air electric floor fan was damaged and repaired with masking tape, exposing employees to an electrical hazard.
