29 CFR 1926.1411(b)(4): If any part of the equipment while traveling will get closer than 20 feet to the power line, the employer must ensure that a dedicated spotter who is in continuous contact with the driver/operator is used. Utility Pole 19 - On or about 8/22/12, the extended boom of a National truck crane struck telecommunication lines and toppled powerlines while the crane traveled around the Howland traffic circle. The crane's boom was elevated less than 12 feet below 12500 volt powerlines and a dedicated spotter was not utilized.

29 CFR 1926.1417(a): The employer must comply with all manufacturer procedures applicable to the operational functions of equipment, including its use with attachments. In the following instances the employer did not comply with the manufacturer procedures applicable to crane traveling: a) National Truck Crane - The boom was not placed securely in the boom rest. b) National Truck Crane - The loadline block was not secured.
