Trucks with a fuel system leak or any other safety defect shall not be operated.    a) An employee was observed operating a powered industrial truck (5th wheel) with a cracked side mirror. The powered industrial truck is labeled as Ottawa 207.    b) An employee was observed operating a powered industrial truck (5th wheel) with bald tires.
