29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) South Side of Building - On or about August 20, 2012 employees were exposed to being struck by falling objects, such as, hammers. Employees working on ground below scaffold were not wearing hard hats.

(a) South Side of Building - On or about August 20, 2025 employees were exposed to fall hazards while working from a scaffold over 13 feet in height. Scaffold did not have a fully planked working level.
