29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toe board wherever, beneath the open sides.  On or about August 22, 2012, employees were installing the roof decking and asphalt shingles to the roof of a mobile home being built while working near the edges of the building and not provided fall protection, exposing employees to a fall hazard of up to approximately15 feet onto a concrete floor.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that every open-sided floor or platform 4 feet or more above adjacent floor or ground level will be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing will be provided with a toe board wherever, beneath the open sides.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade was not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed.  On or about August 22, 2012, employees in the Ceiling Building department were using a Delta radial arm saw with an 8 inch saw blade, which was not provided with a guard on the lower portion of the saw blade, exposing employees to the hazard of coming in contact with the saw blade.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the sides of the lower exposed portion of the blade will be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed.

29 CFR 1910.213(h)(4): Installation shall be in such a manner that the front end of the unit will be slightly higher than the rear, so as to cause the cutting head to return gently to the starting position when released by the operator.  On or about August 22, 2012, two Delta radial arm saws, one in the Saw Mill department and the other in the Exterior Bottom department, were positioned in a manner that didn't allow for the cutting head of the saw to return to the starting position, exposing employees to the hazard of coming in contact with the saw blade.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each radial arm saw is installed in such a manner that the front end of the unit will be slightly higher than the rear, so as to cause the cutting head to return gently to the starting position when released by the operator.
