29 CFR 1926.552(d):     Permanent elevators under the care and custody of the employer and used by employees for work covered by this Act did not comply with the requirements of American National Standards Institute A17.1-1965 with addenda A17.1a-1967, A17.1    ANSI A17.1 1965, Section 101- Machine Rooms and Machinery Spaces, Rule 101.3d: Such doors (machine room access doors) shall be kept closed and locked except during periods when a qualified attendant is on duty in the room or space.    a) Jobsite: Elevator Machine Room, 437 Chestnut Street, Philadelphiia - On or about 8/23/2012, employees were exposed to elevator machine room hazards when the elevator machine room door under the care and custody of Intech Construction was not locked and was propped open with no qualified attendant on duty or in the room.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
