29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    Employees shingling on a residential roof were exposed to fall hazards of up to 20.4 feet.  No fall protection was utilized.

29 CFR 1926.503(a)(1):     The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    Employees installing shingles on a residential single family home were exposed to fall hazards of up to 20.4 feet and the employer had not provided a training program that enabled employees to recognize fall hazards.  Fall protection was not utilized.

29 CFR 1926.20(b)(1):     It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.    The employer had employees exposed to fall hazards and struck-by hazards in the eyes, on a residential construction site in Peoria, Illinois.  The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employees activities.

29 CFR 1926.20(b)(2):     Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    The employer had employees exposed to fall hazards and struck-by hazards in the eyes, on a residential construction site in Peoria, Illinois.  The employer did not have a competent person at the construction site, who was competent in safety rules and requirements, in order to conduct frequent and periodic safety inspections at the construction site.
