29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On August 22, 2012, an employee was exposed to overhead hazards while working below employees on the roof, exposing him to a struck-by hazard.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    On August 22, 2012, an employee was not protected from a fall while working from an aerial lift, exposing the employee to a fall of over six feet.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On August 22, 2012, employees were not protected from falls while walking / working from the roof of a commercial building, exposing the employees to a fall hazard of  19 feet.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about August 22, 2012, employees were required to work from the roof of a commercial property and were not trained to recognize or minimize the hazards associated with a fall.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
