Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: (a) On or about August 14-15, 2012, for the employee that was injured from an uncontrolled well release when he was not provided a means of fast emergency exit from the derrickman's working platform to a safe place at the well site located at the intersection of 125th and 48th Street NW, Williston, North Dakota. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to install the Geronimo Cart on the Geronimo Line or other safety slide device to rapidly exit the derrickmans' platform away from the drilling rig during an uncontrolled release of oil or natural gas as specified in API Recommended Practice 54, Third Edition (Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations), section 6.10 AUXILIARY ESCAPE 6.10.1.

Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: (a) On or about August 14-15, 2012, for the four employees who were exposed to fire and explosion hazards when two trucks were driven through an oil plume at the intersection of 125th and 48th Street NW, Williston, North Dakota. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to remove employees from the hazard zone while the well is releasing oil and gas or to follow the requirements of API Recommended Practice 54 for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, specifically under section 7, Fire Prevention and Protection.

Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the requirements of API Recommended Practice 54 for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations and API Recommended Practice 53 for Blowout Prevention Equipment Systems for Drilling Wells.
