29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. (a) Erickson Metals of Colorado, 5045 Oakland Street, Denver, CO 80239; On and before August 23, 2012, the exposing employer did not ensure that a Voss Engineering Company metal line leveling and shearing machine, serial number 7783, type 66/1.750-19/7-4H-RH, was adequately guarded against accidental contact with the large sheets of aluminum as they fed into the machine. Condition exposed employees operating the machine or in the area to laceration hazards as aluminum sheet is fed into the machine. (b) Erickson Metals of Colorado, 5045 Oakland Street, Denver, CO 80239; On and before August 23, 2012, the exposing employer did not ensure that a Voss Engineering Company metal line leveling and shearing machine, serial number 7783, type 66/1.750-19/7-4H-RH, was adequately guarded against accidental contact at three points where rotating chains integral to the machines operation were located. Two of the chains were located on the side on which the operator faced, one was located in the rear of the machine, all were between the machines planing function and the shearing portion of the machine. Condition exposed employees to the hazard of nip points and rotating parts while the machine is in operation.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury was not guarded, or so designed to and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) Erickson Metals of Colorado, Inc. 5045 Oakland Street, Denver, CO 80239; On and before August 23, 2012, the exposing employer did not ensure that the carriage (part number ASM 121-48-0-A-16-NB/10, serial number 123538) portion of the metal Saw model number CS5 was adequately guarded or constructed to prevent the operator from having any part of his body in the point of operation. Carriage pushes sheets of aluminum into the Precision metal Saw along two sets of tracks. Condition exposed employees to laceration and amputation hazards while carusel is in motion. (b) Erickson Metals of Colorado, 5045 Oakland Street, Denver, CO 80239; On and before August 23, 2012, the exposing employer did not ensure that a Cincinnati-18, 1812 shearing machine, serial number 38933, was properly guarded in the rear of the machine against accidental contact at the point of operation. Condition exposed employees to point of operation hazards as the machine sheared and expelled product.
