29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports. Employees engaged in applying stucco onto a new family dwelling with the use of scaffold(s) were exposed to fall hazards of approximately 12 feet from the top working platform to the ground below. The employees were exposed in that the working platforms were not completely planked.

29 CFR 1926.451(f)(15): Ladders shall not be used on scaffolds to increase the working level height of employees. An employee engaged in applying stucco onto a new family dwelling with the use of scaffold and ladder was exposed to fall hazards of approximately 12 feet from the top working platform to the ground below. The employee was exposed in that the ladder was on the platforms to increase the working height.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Employees engaged in applying stucco onto a new family dwelling with the use of scaffold(s) were exposed to fall hazards of approximately 12 feet from the top working platform to the ground below. The employees were exposed in that a guardrail system was not in place to keep them from falling.
