29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  At the time of the inspection personnel were performing carpentry operations from a work platform that was not attached to the Skytrak Model 6042 Rough Terrain Forklift being used to hold the platform aloft. Employee(s) were exposed to a fall hazard of approximately twelve (12) feet platform to ground.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  At the time of the inspection carpentry operations were being performed without fall protection from a platform being held aloft by a Skytrack Model 6042 Rough Terrain forklift. The employee was exposed to a fall hazard of approximately twelve (12) feet platform to ground.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  At the time of the inspection the stairway hole leading to the basement did not have a guardrail system installed. Employees were exposed to a fall hazard of approximately ten (10) feet to the floor below.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm) were not equipped with one stairrail system along each unprotected side or edge:  At the time of the inspection the stairway leading from the first floor into the basement did not have a guardrail system installed on either of the unprotected sides. Employees were exposed to a fall hazard of approximately ten (10) feet to the floor below.
