Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards:    (a)  On or about August 14-15, 2012, for the six employees who were exposed to fire and explosion hazards when two trucks were idling on site in close proximity to well operations and were driven through an oil plume at the intersection of 125th and 48th Street NW, Williston, North Dakota.    Abatement Note:  Among other methods, one feasible and acceptable abatement method to correct this hazard is to remove employees from the hazard zone while the well is releasing oil and gas or to follow the requirements of API RP 54 for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, specifically under section 7, Fire Prevention and Protection.

Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards:    (a) On or about August 14-15, 2012, for the six employees exposed to the hazards associated with a wild well situation and were not provided appropriate protection from atmospheric, fire, and struck-by hazards at the intersection of 125th and 48th Street NW, Williston, North Dakota.      Abatement Note:  Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the requirements of API RP 54 for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations and API RP 53 for Blowout Prevention Equipment Systems for Drilling Wells. Such a program should include effective well control procedures and evacuation plans in the event of an uncontrolled well release.
