29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Jobsite located at 6440 N. 66th Street, Omaha, NE: A Luna personnel lifting device had a grounded plug installed on a non-grounded wire. The Luna recharging station is a two wire (hot and neutral) electrical system and should not have a ground type of plug. This item was corrected during the inspection.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Jobsite located at 6440 N. 66th Street, Omaha, NE: A Luna personnel lifting device an electrical cord that had pulled away from the plug. This exposed the conductors within the cord. This should have been inspected and removed from service. This item was corrected during the inspection.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: Throughout the company: The employer failed to provide training as prescribed in the paragraph above in that the employees had not been trained on the hazards of the chemicals that they commonly use including but not limited to Buckeye Sanicare Lemon Quat. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
