29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) Lot 178, residential home project: The employer exposed employees to struck-by hazards as they were not required to wear protective eyewear while working with pneumatic nail guns, nails and skill saws. Employees were installing wood framing on the interior ceiling of the home.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. a) Lot 178, 2nd level deck of residential building: The employer exposed employees to fall hazards of 10.5 feet above the ground due to a portable ladder being used improperly with the side rails extended less than a foot above the landing. The employees were using an adjustable Werner 12 ladder to climb to the second level of the building.
