OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to fall hazards from ladders during machine setup and cleaning operations:  (a) Printing area, Kidder-Stacy press; Employees stood on the top steps, including the top plate and top step, of a modified four-foot fiberglass step-ladder to access the #1 and #6 decks on the central impression area of the press during setup and cleaning activities.  (b) Printing area, Kidder-Stacy press; Employees stood on the top steps, including the top plate and top step, of a modified four-foot fiberglass step-ladder to access the #2 and #5 decks to the analox and the middle roller of the press during setup and cleaning activities.

29 CFR 1910.106(e)(2)(iv)(a): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 F (37.8 C), were not kept in covered containers when not actually in use:  (a) Flammable storage room; 55 gallon containers of solvent based ink were not covered while they were in storage. Some containers were stored without covers and other containers had lids, which were cut open to accommodate pneumatic agitators.

29 CFR 1910.106(d)(4)(iv): Ventilation. Every inside storage room was not provided with either a gravity or a mechanical exhaust ventilation system. Such system was not designed to provide for a complete change of air within the room at least six times per hour:  (a) Chemical storage room; Mechanical ventilation did not change air out at least six times per hour inside the storage room.

29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles, and disposed of daily:  (a) Printing and laminating areas; Flammable/combustible waste containers were not emptied daily.  (b) Laminating area; Flammable/combustible waste was disposed of in plastic open-air containers. Cotton rags soaked with 80/20 Solvent were thrown into plastic garbage cans.
