29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: A. Kirkco, LLC worksite located at 12469 Five Point Rd., Perrysburg, Ohio: On or about August 22, 2012, the employer did not ensure the side rails of the step ladder were extended at least three feet above the landing surface. Two employees were exposed to falls while using the ladder to access the roof.

Kirkco, LLC worksite located at 12469 Five Point Rd., Perrysburg, Ohio: On or about August 22, 2012, the employer did not ensure a 6 ft A-framed stepladder was opened during use. The ladder was in the closed position potentially exposing employees to fall hazards while using the ladder to access the roof.
