29 CFR 1926.350(a)(10): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. a. At the workplace, outside in the yard area, oxygen and acetylene cylinders were stored approximatley nine feet apart.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a. At the pipeline construction site located in Cadiz, Ohio the employer had employees working in a trench approximately 7 feet to 12 feet in depth, without adequate cave-in protection exposing employees to a crushed-by hazard.
