29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer:    a) On August 23, 2012, at this job site, employees were doing concrete breaking operations inside a trench excavation approximately fourteen foot deep by eight foot wide by thirty foot long without having the shoring protective system installed correctly in accordance with the manufacturers tabulated data information in which the sheeting did not meet manufacturers data requirement of the top cylinder shall be a minimum of twelve inches below the top excavation. This condition exposed the employees to a crush hazard.    b) On August 23, 2012, at this job site, employees were doing concrete breaking operations inside a trench excavation approximately fourteen foot deep by eight foot wide by thirty foot long without having the shoring protective system installed correctly in accordance with the manufacturers tabulated data information in which there shall be a minimum of three consecutive shores in a row, at the horizontal spacing indicated (or less), to form a shoring system. This condition exposed the employees to a crush hazard.    c) On August 23, 2012, at this job site, employees were doing concrete breaking operations inside a trench excavation approximately fourteen foot deep by eight foot wide by thirty foot long without having the shoring protective system installed correctly in accordance with the manufacturers tabulated data information in which structural steel tube oversleeves were not used. This condition exposed the employees to a crush hazard.    d) On August 23, 2012, at this job site, employees were doing concrete breaking operations inside a trench excavation approximately fourteen foot deep by eight foot wide by thirty foot long without having the shoring protective system installed correctly in accordance with the manufacturers tabulated data information in which the hydraulic shoring was not spaced as required for C type soil. This condition exposed the employees to a crush hazard.    e) On August 23, 2012, at this job site, employees were doing concrete breaking operations inside a trench excavation approximately fourteen foot deep by eight foot wide by thirty foot long without having the shoring protective system installed correctly in accordance with the manufacturers tabulated data information in which the sheeting did not extend to the bottom of the excavation. This condition exposed the employees to a crush hazard.
