29 CFR 1926.1412(d)(1)(v): The employer did not inspect hooks and latches for deformation, cracks, excessive wear, or damage such as from chemicals or heat:    On or about August 14, 2012, at 3315 Broadway Ave J. Galveston, TX; the employer did not ensure that the crane that was being operated by himself had the safety latch closed. This condition exposed the workers beneath to a struck by falling object (load) hazard. The crew was taking down a commercial sign that was 30 ft high from the ground.

29 CFR 1926.1416(d)(3)(i): Telescopic boom cranes manufactured after February 28, 1992, were not equipped with a device which automatically prevents damage from contact between the load block, overhaul ball, or similar component, and the boom tip (or fixed upper block or similar component). The device(s) must prevent such damage at all points where two-blocking could occur:    On or about August 14, 2012, at 3315 Broadway Ave J. Galveston, TX; the employer did not ensure that the anti two block at the crane was working; this to prevent a disengage from the load. This condition exposed the workers beneath to a struck by falling object (load) hazard. The crew was taking down a commercial sign that was 30 ft high from the ground.
