29 CFR 1926.1126(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): (a) Northland Process Piping Inc., 1500 1st Ave., Greeley, CO 80631: On and before 8/23/12, the employer as the controlling contractor did not determine, in accordance with either paragraph (d)(2) or (d)(3) of this section, the 8-hour TWA exposure for each employee exposed to chromium (VI). Subcontractor employees under the control of Northland Process Piping Inc. perform welding on stainless steel parts which can release chromium (VI). This condition could result in employees wearing inappropriate respiratory protection for the situation and exposure to chromium (VI).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Northland Process Piping Inc., 1500 1st Ave., Greeley, CO 80631: On and before 8/23/12, the employer did not provide effective information and training to employees regarding the health hazards associated with chromium (VI) and other welding fume hazards. Employees perform welding on stainless steel parts which can release chromium (VI) and other metals. The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.1200 of this chapter.
