29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: On or about August 17, 2012, at 16926 Southeast Fwy. in Sugarland, TX; the employer did not ensure that the mobile scaffold used by his employees was fully planked. The workers were installing the front canopy for the building. This condition exposed the employee on the scaffold to a 11 ft fall hazard onto the ground.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews: On or about August 17, 2012, at 16926 Southeast Fwy. in Sugarland, TX; the employer did not ensure that the mobile scaffold used by his employees had the guardrails installed. The workers were installing the front canopy for the building. This condition exposed the employee on the scaffold to a 11 ft fall hazard onto the ground.

29 CFR 1926.451(c)(2)(i): Footings shall be level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: On or about August 17, 2012, at 16926 Southwest Fwy. in Sugarland, TX; the employer did not ensure that the mobile scaffold wheels (footings) was set on a leveled foundation creating a displacement hazard. The mobile scaffold was used by his employee to install the front canopy of the building. This condition exposed the worker to a fall hazard of 11 feet onto the ground.
