29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) On or about August 22, 2012, and times prior thereto, at the 230 Bull St. jobsite, the employer failed to assure strain relief to a flexible extension cord on the floor at the pantry office entrance, with outer insulation pulled back from the male end, exposing employees to electrical hazards not limited to overheating, fire and/or electric shock.
